Job description

  • Assists patients with activities of daily living, provides for personal care, emotional support and performs more complex clinical skills under the direction of a RN
  • Function actively in a self-managed work team to coordinate daily care tasks, life engagement, and care of the household
  • Record vital signs, weights, bowel records, meal intake, admission paperwork, and other required documentation
  • Function as an active member of the self-managed work team to coordinate daily care tasks, ensure continuity of care/care givers, promote quality of life, and address the plagues of loneliness, helplessness and boredom
  • Assists the RN in assessment process by obtaining accurate vital signs, height and weight and documents in the medical record appropriately (graphic sheets, I & O)
  • Supporting the residents physical, spiritual, emotional and clinical needs
  • Assisting residents in activities of daily living
  • Implementing residents’ personal care plans, focusing on residents strengths, preferences and preferred routines, while providing a sense of comfort, companionship and belonging
  • Providing assistance with resident activities and resident programs
  • Working with the clinical team to ensure the needs of our residents are being addressed in a timely matter

SKILLS, CERTIFICATION AND REQUIREMENT

  • Maintains an environment conducive to patient comfort and safety
  • Exercises proper care in the use of equipment and reports need to repair to the appropriate person
  • Adheres to fall precaution protocol
  • Adheres to constant observation protocols as assigned
  • Engaging in life-long learning to expand capacity of self and others to create and sustain a learning organization and a purposeful manner of behavior
  • Meets and maintains competency requirements accoding to the nursing credentialing policy procedures
  • Maintains a pattern of reliable attendance, punctuality and completion of timely work assignments
  • Responds willingly to the special needs of the department
  • Utilizes time, equipment, and supplies in a cost-effective manner
  • Participates in unit activities including meetings and projects such as Quality Assurance and Improvement

SKILLS AND CERTIFICATIONS

  • CNA License
  • Clean background check
  • Adhere to HIPPA and patient confidentiality
  • Previous experience in a clinical setting preferred
  • Cardiopulmonary Resuscitation Certification (CPR) required within 30 days of hire
  • High School graduate or GED equivalent with average reading, writing and oral communications skills
  • Licensed as a Certified Nursing Assistant (CNA) in the State of Colorado at time of hire .
  • Previous clinical/patient related experience
  • Must possess and maintain a current Certified Nursing Assistant (CNA) issued by the State of Colorado.
